If you are new to West Spartan Animal Hospital, please print, complete, and bring the following forms to your pet’s appointment.

Please come right into the office for your exam appointments.
Protocol for reopening:
Clients are encouraged to wear a mask when entering the hospital.
Only one client per pet allowed inside during the visit.
Clients will be asked to use hand sanitizer upon their arrival.
Any known positive client is not allowed to enter the hospital without a negative test.
Please do not come into the office if you are sick with symptoms of any kind.
Clients are required to stand on the footprints on the floor.
During the exam, clients will be asked to remain in their seats at all times to protect our technicians and veterinarians.
We encourage all medication pick up, food pick up, and boarding pick up to remain curbside. Remember, we offer Home Delivery for free for all medications and preventatives.
When there are too many clients in the office for them to remain 6 feet apart, no more clients are allowed inside and will be asked to wait in their car until the number of clients can be managed better.
